Forms and Authorizations

Below is a complete list of all forms and authorizations that must be completed prior to the first day of school for your child’s enrollment. Please contact the school office if you have any questions.

  • Required by New York State Office of Children and Family Services, each student must have a fully completed enrollment form.

  • A full medical history is required to be completed by the child’s physician or physician’s office.

  • This form authorizes and approves The Learning Academy to hold your child’s medical records for enrollment

  • This form allows The Learning Academy to take and post pictures of your child to Facebook

  • Some students have special dietary and medical restrictions that prohibit them from consuming or being exposed to nuts. Therefore The Learning Academy is completely nut free. The Food Policy form recognizes that no nut products are allowed on the campus.

  • We file all emergency contact information in the event of weather and unforeseen emergencies

  • Our top priority is keeping our students healthy and safe. We kindly request that all parents be mindful of their children and other students when their child is sick.

  • This form authorizes teachers at The Learning Academy to apply sunscreen and other non-medical items to your child (ex: chapstick, Aquaphor, diaper rash cream) IF NEEDED and use/application stated by the parent. * Form must be completed 2x per school year (every 6 months).
